504HealthNet Receives $50,000 Grant from Aetna

504HealthNet is a community-based consortium whose membership is open to organizations that provide primary care or behavioral health services in a community setting in New Orleans.

504HealthNetThe organization announced it has received a $50,000 grant from Aetna aimed at increasing access to primary care and behavioral health services for residents in the area.

Founded in 2008, the collaboration now includes 22 diverse community health service providers who operate 60 service locations and serve over 130,000 patients.

“We’re thrilled that Aetna is supporting us in this important work,” said Susan Todd, executive director of 504HealthNet. “This funding will allow us to continue our work supporting providers, and helping people get the care that they need.”

With the recent grant, 504HealthNet plans to expand the patient centered medical home (PCMH) model, which is focused on “working with the individual patient and a team of providers to develop a care plan that considers both physical and mental health care needs. It includes prevention and wellness strategies, and acute and chronic care.”
